Racer’s Forms

To participate in MGK events, all racers must complete and submit the following forms prior to racing:

1. Minor Release Waiver (Required for participants under 18)

Slinger Super Speedway will provide these waivers to be filled out BY BOTH PARENTS and the racer in front of a notary or Slinger Speedway staff.

• The waiver must be notarized.
• To arrange notarization, contact Lynn Wagner at slingersuperspeedwaymarketing@gmail.com.

2. Racer-Parent Information Form

• Racer-Parent Form #1
• Complete fully and submit at the MGK trailer or designated area at Slinger Speedway.

3. Emergency Medical Form

• Download Emergency Medical Form
• Complete fully and submit at the MGK trailer or designated area at Slinger Speedway.
